Student placement information form

This form is for supervisors to complete for each of their ID students. It gives the ID team details about the student’s living arrangements, the tasks they will be undertaking, the name of their mentor and how you can be contacted. This information will be helpful to the ID team should we need to get in touch regarding a pastoral concern during the training weeks, or generally to check in about how they’re getting on.

Supervisor and mentor information

The ‘supervisor’ is the person responsible for the day-to-day management of the ID student’s voluntary service to the church. The ‘mentor’ is the primary person who disciples the ID student through the year, asks questions about their study and training and helps to support them pastorally. The supervisor and mentor may be the same person, but more often than not it will be two different people. We recognise that students will be discipled by many in the church through the ID year, nevertheless it’s helpful to name one specific person who commits to regularly check in with them.
